I have sent the following message on Iris Parentmail, unfortunately I cannot attach both forms to school jotter so you will need to access them from your Iris Parentmail Account.

Dear Parents and Carers

Please find attached information regarding the flu vaccines for Reception to Year 6 this autumn term, the date of the immunisation is 18th November 2024.

Please use the link or QR code from the letter, or request a paper copy from the office. Papers copies need to be returned by Wednesday 18th September. The team need a response even if you do not wish your child to have the vaccine.
