Dear Parents and Carers

Please see the information below regarding immunisations from September, as the immunisation team are changing to an electronic based returns system.

We will email a hard copy in September with both the covering letter and consent form for any parents/carers who are unable to complete electronically.

If you have any difficulties please contact the immunisation team who will be happy to help.
